Koç Üniversitesi web forms can be used to make contact forms, short surveys and polls, instructor evaluations, and other forms, free of charge. No knowledge of HTML, JavaScript, or SQL is required.
- Easy web-based form building interface that allows you to preview the form as you're designing it.
- Forms can be embedded within other web pages.
- Forms can be protected via Single-Sign-On (SSO).
- Auto-fill Name and Email address is available if the form is secured via SSO.
- Data can be downloaded to CSV or Excel formats for external analysis/processing.

This service is available at no charge for the KU community.
- KU community members who want a form should fill out the Form Request Form.
- Submit a Trackit request and allow two business days for your request to be completed.
